Service description
The National Office for Reducing Medication Errors (No HARMe) has served as the national database for medication errors since 2014, launching its second version in 2017. In 2022, we are proud to introduce the third version, designed to enable healthcare providers to report medication errors that may occur during the use of pharmaceuticals, biological products, vaccines, cosmetics, or medical devices through simplified steps.
A specialized team from the Egyptian Drug Authority, represented by the General Administration of Drug Utilization and Pharmacy Practice - Central Administration for Pharmaceutical Care, investigates the incident, assesses its risk, and follows up on potential outcomes. The team may issue a report summarizing the incident and proposing corrective actions to prevent recurrence.
Medication errors can occur at any stage of drug management, including prescribing, transcribing, dispensing, preparing, administering, and monitoring. Reporting medication errors is crucial and considered a primary step in the quality assurance measures taken to ensure patient safety and the quality of pharmaceutical care provided.
Confidentiality of data
All information contained in the report is treated with the utmost confidentiality and is fully protected by the Egyptian Drug Authority.
This includes the identity of the patient and the reporter.
